Early detection is key in the fight against chronic kidney disease (CKD) and its root causes such as hypertension and diabetes. Through its chronic disease prevention program, Bridge of Life (BOL) provides health screenings and prevention for at-risk populations around the globe.
Through real-time, lab-quality testing, BOL identifies individuals who have signs of kidney impairment and offers health education sessions based on individual risk factors while empowering patients to take a proactive role in their own health through behavior and lifestyle changes.
BOL screenings include:
- a health survey
- body mass index, blood pressure, urinalysis, serum creatinine for renal function, and blood glucose levels
- a consultation with a nephrologist for high-risk patients
- culturally appropriate health education materials about chronic disease prevention
BOL appreciates your interest in becoming a Chronic Disease Prevention Partner and welcomes the opportunity to discuss potential collaborations within your community. Organizations seeking support should complete the application above. BOL will contact applicants after review of initial applications.
